Before your initial visit, I will email you some paperwork to complete and return to me in advance of our first session including:
- Neurotransmitter assessment sheet
- Nutritional assessment questionnaire
- 3-day food journal
I will then computer score the questionnaire and review your information before you arrive so we can hit the ground running.
